A Case of Hyperinsulinism due to Islet Cell Hyperplasia of


Abstract
This is a case report of hyperinsulinism in 49-year-old female with 8 months history of symptomatic hypoglycemia and documented elevation of fasting plasma insulin to glucose ratio, caused by islet cell hyperplasia of the panceras. Localization
studies,
such as abdominal ultrasonogram, spiral CT scanning, selective superior mesenteric angiography and intraoperative ultrasonogram did not reveal any tumor in the pancreas. Preoperative blood sampling by percutaneous transhepatic portal vein
catheterization showed increased insulin gradient aring from the head of pancreas. Near-total panceratectomy was performed under the presumptive diagnosis of occult insulinoma, but pathological diagnosis was islet cell hyperplasia of pancerea.
The
postoperaitve course was uneventful with disappearance of hypoglycemic symptoms and discharged with no specific operative complication and exocrine dysfunction.
